ScienceDaily (Nov. 24, 2011) — Research on physicians' decision-making processes has revealed that those who pay attention to failures as well as successes become more adept at selecting the correct treatment. The researchers also found that all the physicians in the study included irrelevant criteria in their decisions about treatment.

Gem-interesting article -thanks-some thoughts come to mind-
I can see how in a training session when a student is shown a techniique and he applies it and it does not work-stop and go through the 3 questions mentioned and find the right way. this may help a slow learner to the technique-but---i personally have the student do the technique-then- tell and have the student do the technique again  and if while applyling it hesitates or makes a wrong movement i tell them not to stop but to keep on going to overcome. Then--we go back and see what happened-my rational is that if they stop in the school theu may stop during a real attack---i may have to initially have the techniques done a few times first..I find this method of teaching is a little better than showing a technique once-have the student do it and go to another
